ABSTRACT

Fraction absorbed (Fa) is an important parameter to describe the absorption level of oral drugs, and an important basis for the development and optimization of the formulation process. Because it is easily confused with the concept of absolute bioavailability, it has not received enough attention from the industry. There are many complex factors affecting Fa. There are three time-related factors that directly affect the extent of Fa: the release time, the absorption time, and the residence time. The relationship between these three time-related factors determines the extent of Fa. Generally, we are more concerned about the apparent factors that affect the extent of Fa, including independent variables and covariates; The independent variables include administered dose, route, dosage form, etc. The covariates are divided into internal and external factors, and external factors include food factors, drug interactions, etc. Internal causes include age, sex, disease, etc. This paper analyzes and systematically combs how independent variables and covariates directly or indirectly affect the three time-related factors by affecting the body's physiology and internal environment, thus changing the complex process of Fa. Understanding this theoretical framework can better optimize the independent variables to reduce the impact of covariates on Fa. In addition, this paper also introduces the latest progress of prediction and evaluation of Fa, including the progress of complex dissolution device and the status of software prediction.

ABSTRACT

Objective:To investigate the value of nomograms based on clinical parameters, apparent diffusion coefficient (ADC) and MRI-derived radiomics in predicting survival of patients with locally advanced cervical cancer (LACC) after concurrent chemoradiotherapy (CCRT).Methods:Clinical data of 423 patients with IB-IVA cervical cancer treated with CCRT at Anhui Provincial Hospital Affiliated to Anhui Medical University from March 2014 to March 2020 were retrospectively analyzed and randomly divided into the training and validation groups at a ratio of 2∶1 using the simple randomization method. The values of ADC min, ADC mean, ADC max and 3D texture parameters of diffusion weighted imaging (DWI), T 2WI, T 2WI-fat suppression of pre-treatment primary lesions in all patients were measured. The least absolute shrinkage and selection operator (LASSO) algorithm and logistic regression analysis were used to screen the texture features and calculate radiomics score (Rad-score). Cox regression analysis was employed to construct nomogram models for predicting overall survival (OS) and cancer-specific survival (CS) of patients with LACC after CCRT, which were subject to internal and external validation. Results:Squamous cell carcinoma antigen (SCC-Ag), external beam radiotherapy dose, ADCmin and Rad-score were the independent prognostic factors for OS and CS of LACC patients after CCRT and constituted predictive models for OS and CS. The area under the receiver operating characteristic (ROC) curve (AUC) of two models in predicting 1-year, 3-year, 5-year OS and CS was 0.906, 0.917, 0.916 and 0.911, 0.918, 0.920, with internally validated consistency indexes (C-indexes) of 0.897 and 0.900. Then, models were brought into the validation group for external validation with AUC of 0.986, 0.942, 0.932 and 0.986, 0.933, 0.926 in predicting 1-year, 3-year, 5-year OS and CS.Conclusion:The nomograms based on clinical parameters, ADC values and MRI-derived radiomics are of high clinical value in predicting OS and CS of patients with LACC after CCRT, which can be used as prognostic markers for patients with cervical cancer to certain extent.

ABSTRACT

Objective:To investigate the value of conventional MRI and high resolution diffusion weighted imaging (DWI) for preoperative discrimination between nasopharyngeal-skull base osteomyelitis (NP-SBO) and locoregionally advanced nasopharyngeal carcinoma (LA-NPC).Methods:From January 2017 to October 2021, 27 patients of NP-SBO and 32 patients of LA-NPC were retrospectively analyzed at the Eye & ENT Hospital of Fudan University. The clinical characteristics and conventional MRI features were collected, and the apparent diffusion coefficient (ADC) values of polygonal (ADC polygonal) and small circle were measured from readout segmentation of long variable echo-trains (RESOLVE) DWI. MRI features included laterality, margin, signal intensity of T 1WI and T 2WI, enhancement degree, component, abscess, deep mucosal white line, bone invasion, lymph nodes involvement and other accompany symphtoms. The independent sample t test, χ 2 test or Fisher exact test were used to compare the features and ADC values of the NP-SBO and LA-NPC groups. The logistic regression was applied to select independent predictors in the distinguishing LA-NPC from NP-SBO. Then, the conventional MRI model, ADC model and conventional MRI in combination with ADC model were built. The area under the receiver operating characteristic curve (AUC) of models were compared using DeLong test. Results:The age, diabetic status, cranial nerve deficits, inner component, abscess, deep mucosal white line, lymph nodes involvement and ADC polygonal were significantly different between NP-SBO and LA-NPC groups ( P<0.05). The logistic regression analysis showed that ADC polygonal (OR=0.972, 95%CI 0.951-0.993, P=0.011) and abscess (OR=0.101, 95%CI 0.013-0.774, P=0.027) were the independent predictors in the discrimination of NP-SBO and LA-NPC. The AUC (95%CI) of conventional MRI model (abscess), ADC model (ADC polygonal) and combination model were 0.634 (0.499-0.756), 0.870 (0.757-0.943), and 0.925(0.829-0.979), respectively. The AUC of combination model was higher than that of conventional MRI model ( Z=4.77, P<0.001), while there was no difference between combination model and ADC model ( Z=1.87, P=0.062). The AUC of conventional MRI model was lower than that of ADC model ( Z=2.84, P=0.005). Conclusion:Conventional MRI in combination with RESOLVE DWI shows good performance in differentiating between NP-SBO and LA-NPC, especially for abscess in combination with ADC polygonal value.

ABSTRACT

OBJECTIVES@#Magnetic resonance diffusion-weighted imaging (DWI) has important clinical value in diagnosis and curative effect evaluation on endometrial carcinoma. How to improve the detection rate of endometrial small lesions by DWI is the research focus of MRI technology. This study aims to analyze the image quality of small field MRI ZOOMit-DWI sequence and conventional single-shot echo-planar imaging (SS-EPI) DWI sequence in the scanning of endometrial carcinoma, and to explore the clinical value of ZOOMit-DWI sequence.@*METHODS@#A total of 37 patients with endometrial carcinoma diagnosed by operation and pathology in the Second Xiangya Hospital of Central South University from July 2019 to May 2021 were collected. All patients were scanned with MRI ZOOMit-DWI sequence and SS-EPI DWI sequence before operation. Two radiologists subjectively evaluated the anatomical details, artifacts, geometric deformation and focus definition of the 2 groups of DWI images. At the same time, the signal intensity were measured and the signal-to-noise ratio (SNR), contrast to noise ratio (CNR), and apparent diffusion coefficient (ADC) of the 2 DWI sequences were calculated for objective evaluation. The differences of subjective score, objective score and ADC value of the 2 DWI sequences were analyzed.@*RESULTS@#The SNR of the ZOOMit-DWI group was significantly higher than that of the SS-EPI DWI group (301.96±141.85 vs 94.66±41.26), and the CNR of the ZOOMit-DWI group was significantly higher than that of the SS-EPI DWI group (185.05±105.45 vs 57.91±31.54, P<0.05). There was no significant difference in noise standard deviation between the ZOOMit-DWI group and the SS-EPI DWI group (P>0.05). The subjective score of anatomical detail and focus definition in the ZOOMit-DWI group was significantly higher than that of the SS-EPI DWI group (both P<0.05). The subjective score of artifacts and geometric deformation of ZOOMit-DWI group was significantly lower than that of the SS-EPI DWI group (both P<0.05). ADC had no significant difference between the ZOOMit-DWI group and the SS-EPI DWI group (P>0.05).@*CONCLUSIONS@#The image quality of ZOOMit-DWI is significantly higher than that of conventional SS-EPI DWI. In the MRI DWI examination of endometrial carcinoma, ZOOMit-DWI can effectively reduce the geometric deformation and artifacts of the image, which is more conducive to clinical diagnosis and treatment.

ABSTRACT

Objective:To investigate the Correlation between ADC combined with serum C-reactive protein (CRP) and delayed encephalopathy after carbon monoxide poisoning (DEACMP), It provides scientific basis for early prediction of DEACMP.Methods:According to the design principle of case-control study, the data of acute carbon monoxide poisoning (ACOP) patients admitted to Shandong Provincial Hospital from December 2017 to December 2021 were retrospectively selected. Among them, patients with DEACMP were selected as the case group, without DEACMP were used as the control group. Univariate and multivariate analyses were performed on the two groups. Receiver operating characteristic curve (ROC) was used to evaluate the diagnostic efficacy of ADC combined with CRP as a combined predictor for disease.Results:A total of 89 patients with ACOP were included, including 33 patients with DEACMP and 56 patients without DEACMP. There were no significant differences in gender, age, smoking, drinking, and underlying diseases (hypertension, coronary heart disease) between groups ( P>0.05). Logistic regression analysis showed that white blood cell count (WBC) ( OR=1.64, 95% CI: 1.19-2.26, P=0.003), CRP ( OR=1.22, 95% CI: 1.03-1.45, P=0.019) and ADC value of central semiovale white matter ( OR=0.99, 95% CI: 0.98-1.00, P=0.010) were associated with DEACMP in patients with ACOP. The ROC curve results showed that the area under the ROC of ADC combined with CRP in the center of semiovale was 0.765 (95% CI: 0.656-0.845), the specificity was 87.9%, the sensitivity was 23.2%, and the cut-off value was 3.5°. Conclusions:WBC, CRP and ADC value of central semiovale are independent factors for DEACMP. ADC value of central semiovale combined with CRP has more clinical value in the early diagnosis of DEACMP. For ACOP patients with DEACMP triggering factors, the diagnosis and treatment awareness of early screening of brain magnetic resonance imaging should be strengthened to avoid DEACMP.

ABSTRACT

Objective:To evaluate the therapeutic effect of hemopurification on acute chlorfenapyr poisoning according to the blood concentration of chlorfenapyr and to provide experience for clinical treatment.Methods:Two patients who presented to our Emergency Department following an ingestion of chlorfenapyr and then were treated with hemopurification in 2022 were included. The concentrations of chlorfenapyr and its highly toxic metabolite tralopyril were dynamically monitored, and the clinical data of the patients were collected.Results:Case 1 was given hemoperfusion for the first time 13 hours after ingestion. During l hour hemoperfusion, the tralopyril decreased by 28.82%. The concentration increased and exceeded the pre-perfusion level after 2 hours of hemoperfusion. After three times of hemoperfusion, the concentrations of chlorfenapyr and tralopyril were still higher than those before the first time, reaching 248 ng/mL and 1 307 ng/mL respectively. The concentration of chlorfenapyr showed a downward trend after 130 h, and the tralopyril in blood reached the peak 3 164 ng/mL at 130 h and decreased to 2 707 ng/mL at 178 h. In case 2, the blood chlorfenapyr and tralopyril concentration was 392 ng/mL and 7 598 ng/mL respectively 150 hours after ingestion. The blood chlorfenapyr concentration decreased by 37.75% respectively after first hemoperfusion, and the tralopyril concentration decreased by 38.02% respectively. During 85 hours of continuous veno-venous hemodiafiltration (CVVHDF), the concentration of tralopyril was maintained at 4 234~6 410 ng/mL. Case 1 was followed up to 12 days and lost follow-up. Case 2 died and the survival time was 247 hours.Conclusions:Hemoperfusion can scavenge tralopyril, but CVVHDF has poor scavenging ability for tralopyril. And the apparent volume of distribution (Vd) of chlorfenapyr and tralopyril are large. After ingestion, chlorfenapyr spreads to various tissues quickly, and it is easy to accumulate in the adipose tissue. The chlorfenapyr in the tissue slowly is released back to the blood and stays in the blood for a long time. The peak concentration of chlorfenapyr appeared earlier than that of tralopyril. Clinicians should pay attention to the early removal of toxins from the digestive tract.

ABSTRACT

ObjectiveTo assess the value of apparent diffusion coefficient (ADC) in the treatment of uterine fibroid using magnetic resonance guided focused ultrasound surgery (MRgFUS). MethodsThe MRI and clinical data of 56 patients with uterine fibroid before, at 3 and 6 months after MRgFUS treatment, at Foshan Hospital of Traditional Chinese Medicine from December 2018 to October 2022, were retrospectively analyzed. The correlation between the ADC value and lesion volume, symptoms severity score (SSS) and uterine fibroid symptoms quality of life questionnaire (UFS-QOL) were analyzed. ANOVA was used to compare the differences in related parameters before and after treatment, and Pearson’s method was performed to analyze data correlation. ResultsThere were significant differences in ADC value [(1.11±0.13), (1.84±0.09), (2.12±0.24),×10-3/(mm2/s)], lesion volume (102±35.30, 56.70±18.88, 46.93±18.99,cm3), SSS (36.73±11.74, 21.77±10.21, 17.66±9.30) and UFS-QOL score (59.05±17.48, 76.54±16.50, 82.46±12.37) between before treatment and each time point after treatment (F value was 557.837, 73.589, 53.976 and 37.606, respectively, all P<0.05). The ADC values were negatively correlated with lesion volume and SSS, and positively correlated with UFS-QOL score, with correlation coefficients of -0.586, -0.630 and 0.592, respectively (all P<0.05). ConclusionThe ADC value has clinical significance for the treatment of uterine fibroid using MRgFUS.

ABSTRACT

Objective To investigate the value of MRI diffusion-weighted imaging (DWI) technique in endocrine therapy for prostate cancer (PCa) based on PI-RADSv2.1. Methods A retrospective analysis of 57 patients with pathologically confirmed PCa was conducted. All patients underwent multi-parametric MRI (mpMRI) according to PI-RADS v2.1 technical specifications before biopsy and six months after endocrine therapy. The apparent diffusion coefficient (ADC) values were measured in cancer and non-cancer areas before biopsy and six months after endocrine therapy. Patients were grouped based on the mRECIST criteria and PSA level into responders (n=45) and non-responders (n=12). ROC curves were obtained to assess the correlation between changes in ADC values and PSA values before and after endocrine therapy. Results In the responder group, the ADC value of the cancer areas was increased significantly after endocrine therapy (P<0.001). No statistically significant difference of the ADC value of the cancer areas was found in the non-responder group before and six months after endocrine therapy (P=0.714). The ADC change of responders and non-responder groups were (0.411±0.178)×10-3 mm2/s and (-0.014±0.125)×10-3 mm2/s, respectively (P<0.001); the ADC ratio were (60.603±30.201)% and (-1.096±13.175)%, respectively (P<0.001). The cutoff value of the ADC change was 0.165 (AUC=0.974; sensitivity, 88.89%; specificity, 100.00%; PPV, 100.00%; NPV, 70.59%). The cutoff value of ADC ratio was 16.827% (AUC=0.980; sensitivity, 91.11%; specificity, 100.00%; PPV, 100.00%; NPV, 75.00%). The ADC values were negatively correlated with serum PSA before and after endocrine therapy. Conclusion The ADC change and ADC ratio may be facilitated to monitor the efficacy of endocrine therapy for PCa. The ADC values were negatively correlated with serum PSA.

ABSTRACT

ObjectiveTo investigate the value of reduced field-of-view DWI (r-FOV DWI) in quantitative assessment of axial spondyloarthritis. MethodsA total of 112 patients with chronic back pain or suspected axial spondyloarthritis receiving full field-of-view DWI (f-FOV DWI) and reduced field-of-view DWI (r-FOV DWI) from December 2019 to December 2021 were enrolled. Next, subjective image quality assessment (anatomical detail, artifacts, distortion, overall image quality) and objective image quality assessment including (signal to noise ratio and contrast to noise ratio) were conducted by two experienced radiologists. In addition, the Apparent Diffusion Coefficient (ADC) values of three groups (active group, inactive group, and control group) on the two DWI sequences were measured by the two radiologists, respectively. Finally, the consistency of measurement between the two researchers was evaluated and the differences in ADC values was compared. Results102 patients were included and were divided into three groups, including the active group (n=32), inactive group (n=29), and control group (n=41) according to ASAS diagnostic criteria. All subjective and objective image quality metrics were rated in favor of r-FOV DWI images compared with f-FOV DWI images [overall image quality: DWI 4(3~4) vs. 3(3~3) and SNR: 6.58(5.05~10.38) vs. 4.46(2.37~10.04), CNR: 2.04(-1.14~8.29) vs. 0.97(-8.19~7.12);P<0.05]. Inter-rater consistency of the two researchers were 0.60~0.74. According to the AUC curve, group inactive vs. control showed r-FOV DWI was better than f-FOV DWI. In other groups (lesion vs. control, active vs. inactive), there were no differences between both sequences(P<0.05). ConclusionThe subjective image quality score and signal to noise ratio of r-FOV DWI were higher than those of f-FOV DWI, which could be used for quantitative assessment of axial spondyloarthritis.

ABSTRACT

【Objective】 To predict the expression of human epidermal growth factor receptor 2 (HER2) in urothelial bladder carcinoma based on normalized apparent diffusion coefficient (ADC). 【Methods】 The preoperative pelvic 3.0T magnetic resonance imaging (MRI) images of 127 patients with urothelial bladder carcinoma were retrospectively studied, the ADC was measured, and the HER2 expression in postoperative tissue specimens was determined with immunohistochemistry (IHC). The differences in normalized ADC were analyzed among different HER2 expressions and among different expression divisions. Correlation between normalized ADC and HER2 expression was analyzed. The optimal diagnostic threshold for distinguishing different expression divisions were determined with receiver operating characteristic (ROC) curve. 【Results】 Normalized ADC was negatively correlated with HER2 expression (tau-b=-0.180, P=0.008). Normalized ADC of HER2 overexpression group (IHC 2+, 3+) was lower than that of HER2 negative group (IHC 0, 1+) (P=0.081). Normalized ADC of HER2 expression group (IHC 1+, 2+, 3+) was significantly lower than that of HER2 zero-expression group (IHC 0) (P=0.020). Normalized ADC of HER2 strong positive group (IHC 3+) was significantly lower than that of HER2 non-strong positive group (IHC 0, 1+, 2+) (P=0.024). The optimal diagnostic threshold of HER2 strong positive group was 0.849; the sensitivity, specificity and accuracy were 0.621, 0.909 and 0.765, respectively. The optimal diagnostic threshold of HER2 overexpression group was 0.909; the sensitivity, specificity and accuracy were 0.547, 0.667 and 0.607, respectively. 【Conclusion】 Normalized ADC is negatively correlated with HER2 expression. ADC may be a potential marker for predicting HER2 expression.

ABSTRACT

Objective:To investigate the diagnostic value of diffusion kurtosis imaging (DKI) quantitative parameters in lymph node metastasis of rectal cancer.Methods:The clinicopathological data of 79 patients with rectal cancers in Shanxi Province Cancer Hospital from November 2016 to March 2017 were retrospectively analyzed. All patients underwent routine magnetic resonance image (MRI) sequence and DKI sequence examinations before the operation. The tumor region of interest (ROI) was delineated by two radiologists. Matlab software was applied to calculate DKI quantitative parameters including apparent diffusion coefficient (ADC), mean diffusivity (MD) and mean kurtosis (MK) in two groups, respectively; and the consistency analysis was performed by using the interclass correlation coefficients (ICC). According to the results of postoperative pathology, all patients were divided into lymph node metastasis group and lymph node non-metastasis group; ADC, MD, MK of both groups were compared. The pathological diagnosis results were taken as the gold standard, receiver operating characteristic (ROC) curves of DKI quantitative parameters in the diagnosis of lymph node metastasis of rectal cancer were plotted, the area under the curve (AUC) was calculated, the optimal diagnostic threshold was determined based on the Yoden index, and the sensitivity and specificity were calculated.Results:The ICC of ADC, MD and MK calculated by two physicians were 0.934 (0.833-0.975), 0.963 (0.905-0.981) and 0.971 (0.949-0.991), respectively, showing a good inter-observer consistency. Among the 79 patients with rectal adenocarcinoma, 36 cases were in lymph node metastasis group and 43 cases were in lymph node non-metastasis group. MK value in lymph node metastasis group was higher than that in lymph node non-metastasis group, and the difference was statistically significant (0.97±0.08 vs. 0.89±0.09; t = -4.07, P < 0.001), while the ADC and MD values in lymph node metastasis group were lower than those in lymph node non-metastasis group, and the differences were not statistically significant (all P > 0.05). The AUC of MK value in the diagnosis of lymph node metastasis of rectal cancer was 0.735, and the corresponding sensitivity and specificity were 55.56% and 88.37%, respectively. Conclusions:DKI quantitative parameter MK has a certain diagnostic value in predicting lymph node metastasis of rectal cancer.

Resumen Introducción: La imagen ponderada en difusión es una técnica de imagen funcional que aporta información complementaria a las secuencias convencionales de resonancia magnética. El coeficiente de difusión aparente (CDA) se valora junto con la secuencia de difusión, calculando el logaritmo negativo de la proporción de intensidades de dos conjuntos de imágenes, lo que permite el análisis cuantitativo de la restricción a la difusión. Objetivo: Determinar si existe una diferencia significativa entre los valores de CDA de las lesiones musculoesqueléticas benignas y malignas, correlacionando los resultados con los hallazgos histológicos. Método: Estudio prospectivo, observacional y descriptivo. Se incluyeron en el trabajo pacientes con sospecha de lesiones tumorales del sistema musculoesquelético de cualquier edad y sexo, sin antecedentes de radioterapia previa o cirugía, evaluados con resonancia magnética y con posterior biopsia o cirugía. Resultados: Se incluyeron 170 pacientes, de los cuales 80 (44 benignos) eran hombres y 90 (43 benignos) eran mujeres. El total fue de 87 lesiones benignas y 83 lesiones malignas, con una media de CDA de 1,44 ± 0,55 × 10−3 mm2/s y de 0,96 ± 0,4 × 10−3 mm2/s, respectivamente, con una diferencia estadísticamente significativa (p < 0,001; 6,335; 168). Conclusiones: Los valores de difusión, y particularmente el CDA, han aportado información adicional en la caracterización de los tumores benignos y malignos, con diferencia significativa, estableciendo valores altos de CDA para los tumores benignos.


Abstract Introduction: Diffusion-weighted imaging is a functional imaging technique that provides complementary information to conventional magnetic resonance imaging sequences. The apparent diffusion coefficient (ADC) is evaluated together with the diffusion sequence, calculating the negative logarithm of the intensity ratio of two image sets, allowing the quantitative analysis of the diffusion. Objective: To determine if there is a statistical significance between the ADC values of benign and malignant musculoskeletal lesions, correlating the results with the histological findings. Method: Prospective, observational and descriptive study. Patients with suspected tumoral lesions of the musculoskeletal system of any age and sex, with no history of prior radiation therapy or surgery, evaluated with resonance with subsequent biopsy or surgery. Results: This study included 170 patients. Male patients were 80 (44 benign), and female patients were 90 (43 benign). The total was 87 benign lesions and 83 malignant lesions, with a mean ADC of 1.44 ± 0.55 × 10−3 mm2/s and 0.96 ± 0.4 × 10−3 mm2/s respectively, with a statistical significance (p < 0.001; 6.335; 168). Conclusions: Diffusion and, in particular, ADC values have provided additional information in the characterization of benign and malignant tumors, with a statistical significance, establishing high ADC values for benign tumors.

ABSTRACT

Background: Corn is, quantitatively, one of the most important world crops (ranking second only after wheat) and a key ingredient in animal feeds. Objective: to assess and compare corn quality, mycotoxin content, chemical composition and apparent metabolizable energy (AME) of domestic and imported corn. Methods: Grain quality (USDA grading system) was determined in 30 samples of domestic and 21 samples of imported corn. From each origin, 15 samples were subjected to proximal analysis and 10 were used to determine fatty acid composition. Mycotoxin analysis was conducted on 30 samples of domestic and 23 of imported corn. Results: six of the 30 domestic samples corresponded to US1 grade (highest quality) vs. none of the imported. In the "sample grade" category (lowest quality), 10 and 6 samples corresponded to imported and domestic corn, respectively. Soybeans were found as contaminant in 15 of the 21 imported corn samples. Aspergillus spp. mycotoxins such as ochratoxin A were not detected, and aflatoxins were found in only a few samples at very low levels. Fusariotoxins such as deoxynivalenol and zearalenone were found in 61 and 43% of imported samples, respectively, but in none of the domestic samples. Domestic corn had lower carbohydrate content compared with imported corn (85.4 vs. 86.7%), but higher crude fat (3.8 vs. 3.1%). The AME values for domestic and imported corn were 3,697 and 3,378 kcal/kg, respectively. The fatty acid profiles from both corn types were similar. Conclusion: This study found significant differences between locally-grown and imported corn, particularly in terms of crude fat, AME content, fusariotoxins, and contaminant seeds (soybeans). These findings suggest that locally-grown corn might have nutritional and toxicological advantages over corn imported from the United States.


Antecedentes: El maíz es, cuantitativamente, uno de los cultivos más importantes a nivel mundial (ocupa el segundo lugar después del trigo) y uno de los principales ingredientes en dietas para animales. Objetivo: Evaluar y comparar la calidad del maíz, su contenido de micotoxinas, composición química, y energía metabolizable aparente (AME) del maíz nacional e importado. Métodos: La calidad del grano (sistema de clasificación de la USDA) se determinó en 30 muestras de maíz producido en Colombia y 21 de maíz importado. De cada origen, 15 muestras se sometieron a análisis proximal y 10 se analizaron para determinar el perfil de ácidos grasos. El contenido de micotoxinas se determinó en 30 muestras de maíz nacional y 23 de maíz importado. Resultados: Seis de las treinta muestras nacionales analizadas para calidad de grano correspondieron a grado US1 (la mejor calidad) frente a ninguna de las importadas. En categoría "grado muestra" (la menor calidad) se encontraron 10 y 6 de las muestras de maíz importado y nacional, respectivamente; 15 de las 21 muestras de maíz importado presentaron contaminación con semillas de fríjol soya. No se encontraron micotoxinas de hongos Aspergillus spp. (ocratoxina A), y niveles muy bajos de aflatoxinas en unas pocas muestras. Fusariotoxinas tales como deoxinivalenol y zearalenona se detectaron en el 61 y 43% de las muestras de maíz importado, respectivamente, pero en ninguna muestra de maíz nacional. El maíz nacional presentó menor contenido de carbohidratos (85,4 vs. 86,7%), pero mayor contenido de grasa cruda (3,8 vs. 3,1%) en comparación con el importado, respectivamente. Los valores de AME fueron de 3.697 y 3.378 kcal/kg para el maíz nacional e importado, respectivamente. El perfil de ácidos grasos de ambas procedencias fue similar. Conclusiones: Este estudio encontró diferencias significativas entre el maíz de producción nacional y el importado, en especial en su contenido de grasa cruda y AME, fusariotoxinas, y semillas contaminates (soya). Estos hallazgos sugieren que el maíz de producción nacional puede presentar ventajas de tipo nutricional y toxicológico frente al maíz importado de los Estados Unidos de América.


Antecedentes: O milho é quantitativamente uma das culturas mais importantes em nível mundial (ocupando o segundo lugar depois do trigo), sendo um dos principais ingredientes em dietas para animais. Objetivo: Avaliar e comparar a qualidade do milho, o teor de micotoxinas, a composição química e a energia metabolizável aparente (EMA) do milho nacional e importado. Métodos: A qualidade do grão (sistema de classificação do USDA) foi determinada em 30 amostras de milho produzido na Colômbia e 21 amostras de milho importado. Quinze amostras de cada tipo de milho foram submetidas a análise proximal e dez amostras de cada milho foram analisadas para determinar o perfil de ácidos graxos. O conteúdo de micotoxinas foi determinado em 30 amostras de milho da Colômbia e em 23 amostras de milho importado. Resultados: Seis das trinta amostras colombianas analisadas para qualidade de grão corresponderam a grau US1 (melhor qualidade) frente a nenhuma das amostras importadas. Na categoria "grau amostra" (menor qualidade) foram encontradas 10 e 6 amostras de milhos importados e colombianos, respectivamente; Quinze das 21 amostras de milho importado apresentaram contaminação com sementes de feijão soja. Não foram encontradas micotoxinas de fungos Aspergillus spp. (ocratoxina A) ou apenas níveis muito baixos em poucas amostras (aflatoxinas). As fusariotoxinas, deoxinivalenol e zearalenona foram detectadas em 61 e 43% das amostras de milho importado, respectivamente, mas não foram detectadas em nenhuma amostra do milho colombiano. O milho colombiano apresentou menor conteúdo de carboidratos (85,4 vs. 86,7%) porém maior conteúdo de gordura crua (3,8 vs. 3,1%). Os valores de AME foram 3.697 e 3.378 kcal/kg para as amostras de milho colombiano e importado, respectivamente. O perfil de ácidos graxos de ambos tipos de milho foi similar. Conclusões: Os resultados do presente estudo mostram diferenças significativas entre o milho de origem colombiana e o milho importado dos Estados Unidos, em especial no conteúdo de gordura crua e AME, fusariotoxinas e sementes contaminantes (soja). Estes achados sugerem que o milho de produção colombiana pode apresentar vantagens de tipo nutricional e toxicológica frente ao milho importado dos Estados Unidos.

ABSTRACT

OBJECTIVES@#Reversible posterior leukoencephalopathy syndrome (RPLS) is a rare clinical imaging syndrome. The causes of RPLS are complex and diverse, the pathogenesis is not yet clear. The onset is urgent and the onset age span is large, ranging from children to the elderly. The clinical symptoms of RPLS have no significant specificity, which can be manifested as headache, blurred vision, disturbance of consciousness or seizures. Clinicians have little knowledge on the disease, which may lead to misdiagnosis or missed diagnosis. This study aims to analyze and summarize the MRI changes and clinical characteristics regarding RPLS patients, so as to provide basis for rapid diagnosis and timely intervention for this disease.@*METHODS@#The clinical data and complete imaging data of 77 patients with RPLS diagnosed in Xiangya Hospital of Central South University from January 2012 to March 2021 were retrospectively collected. The main image data include T1 weighted imaging (T1WI), T2 weighted imaging (T2WI), T2 liquid attenuation inversion recovery (T2-FLAIR), diffusion weighted imaging (DWI) (b value=1 000×10-6 mm2/s), and apparent diffusion coefficient (ADC). The case group included 63 patients who underwent DWI examination, and 71 normal controls matched in age and sex. The characteristics of patients' magnetic resonance signals and the ADC value of 19 regions of interest (ROI) were analyzed. The differences in bilateral ADC value in the case group, the difference of ADC value between the case group and the normal control group, and the difference of ADC value in the case group before and after treatment were compared.@*RESULTS@#Compared with the normal control group, the ADC value of the right frontal lobe, bilateral parietal lobe, bilateral thalamus, bilateral head of caudate nucleus, left lenticular nucleus, right internal capsule, bilateral temporal lobe and pons in the case group were significantly higher (all P<0.01). There was no significant difference in ADC value of bilateral sides of the case group and before and after treatment in the case group (all P>0.01). The lesions of RPLS were widely distributed and multiple, usually high signal in the posterior parieto temporo occipital lobe or pons of the brain, and involved the cortex and subcortical white matter. Most of them were bilateral, but not completely symmetrical.@*CONCLUSIONS@#The imaging manifestations of RPLS and the occurrence and development of clinical symptoms are basically synchronous. The imaging manifestations are specific. Magnetic resonance imaging can show the range of involvement of RPLS. ADC value can provide information on the severity of the disease and predict the prognosis. There are few reversible diseases. It is very important to fully understand and timely diagnose the disease.

OBJECTIVE@#To explore the characteristics of ADC value changes in DWI of newly diagnosed symptomatic MM patients and its correlation with R-ISS stage.@*METHODS@#The data of 148 newly diagnosed symptomatic MM patients treated by whole-body DWI scan at The First Affiliated Hospital of Soochow University from June 2016 to June 2019 were selected and retrospectively analyzed and 30 cases of age-matched healthy people were selected as controls. The differences of ADC values between the patients in normal control group, DWI- group and DWI+ group were compared, and the relationship between ADC values and R-ISS stage in MM patients was compared.@*RESULTS@#The plasma cell percentage of the patients in DWI+ group was higher than those in DWI- group. ADC values of vertebra, sternum, rib, pectoral girdle, pelvic girdle of the patients in DWI+ group were significantly higher than those in DWI- group and normal control group. The ADC values of each part of the patients in DWI- group were higher than those in normal control group. ADC values of sternum, rib and pectoral girdle in the patients at R-ISS stage III were higher than those at R-ISS stage I and II, while, there was no statistical difference between R-ISS stage I and II groups. And there was no significant difference in ADC values of other bone parts such as vertebra and pelvic girdle in patients at R-ISS stage Ⅰ-Ⅲ.@*CONCLUSION@#DWI+ in MM patients is related to higher tumor invasion. The ADC values of the DWI+ group are higher than those of the DWI- group; the bone ADC values of the DWI- patients are still higher than the normal ones. And there is a certain relationship between ADC value and R-ISS stage.

ABSTRACT

Objective:To prospectively guide the change of chemotherapy regimen in mouse 5-fluorouracil (5-FU) resistance subcutaneous xenograft tumor model derived from gastric cancer patients by the early changes of MRI apparent diffusion coefficient (ADC), and to compare the difference of tumor load between ADC guided dressing change group and volume guided dressing change group.Methods:From January to June 2020, thirty patient-derived xenografts mouse models were established using 5-FU resistant gastric cancer cells coming from patients, and were randomly divided into experimental group and control group by AdaBoost algorithm, with 15 mice in each group. On the 26th day after transplantation, all mice began chemotherapy with 5-FU as the first-line chemotherapy drug, and underwent MR examination once every two days, including T 2WI and diffusion weighted imaging (DWI). Volumes of tumors were measured using an open-source software ITK-SNAP and values of ADC were measured on ADC maps. According to the change rate of tumor ADC value in the experimental group and the tumor volume growth rate in the control group, the replacement time of chemotherapy drugs was determined, and 5-FU was replaced by paclitaxel. The end point of the experiment was the day that the mice entered the cachexia state. Independent-sample t test was used to compare the difference of tumor load between the two groups. Results:After 5-FU treatment, the ADC value of the two groups both increased. The ADC value began to decline on the 4th day after chemotherapy, and the experimental group continued chemotherapy with paclitaxel instead of 5-FU at this time point. The tumor volume growth rate of the control group increased significantly on the 6th day after chemotherapy (from 8.6% to 16.1%), and the control group used paclitaxel instead of 5-FU chemotherapy at this time point. The observed end point was on the 18th day after chemotherapy. The tumor load of the experimental group [(1.82±0.09) cm 3] was lower than that of the control group [(2.01±0.09) cm 3], and the difference was statistically significant ( t=2.25, P=0.033). On the 16th day after chemotherapy in the experimental group and the 18th day after chemotherapy in the control group, the time of paclitaxel administration in both groups was 12 days. The tumor load in the experimental group [(1.61±0.12) cm 3] was also lower than that in the control group [(2.01±0.09) cm 3], and the difference was statistically significant ( t=2.03, P=0.040). Conclusions:For the subcutaneous transplantation model of 5-FU resistant gastric cancer mice, according to the early changes of tumor ADC value after chemotherapy, the replacement of chemotherapy drugs can obtain a lower tumor load, suggesting that it is a feasible method to optimize the chemotherapy regimen.

ABSTRACT

Objective:To assess the clinical and imaging features of NUT gene-related sinonasal carcinomas (NUT midline carcinome).Methods:The clinical data and pretreatment imaging findings of 5 cases with pathologically proven NUT sinonasal carcinomas were analyzed retrospectively in Beijing Tongren Hospital, Capital Medical University from January 2016 to December 2020. Of 5 cases, the tumors affected 4 females and 1 male with an age range of 15 to 48 years (median 19 years). Clinical data of all cases were available before surgery with both CT and MR examination. Tumor location, CT density, boney change, calcification, tumor size, T 1WI, T 2WI and diffusion weighted imaging (DWI) signal intensity, appearance diffusion coefficient (ADC), type of time intensity curve (TIC) of dynamic contrast-enhanced (DCE)-MRI were evaluated. Results:All five cases belonged to T4 stage of the clinic TNM system. The locations were nasal cavity ethmoid, sphenoid and maxillary sinus ( n=1), nasal and maxillary sinus ( n=1), nasal cavity and ethmoid sinus ( n=3). Iso-attenuated in 3 cases, heterogeneous with local necrosis in 2 cases, and heterogeneous with calcification in 3 cases on CT imaging. Bone erosion was found in 4 cases, and bone erosion with destruction in 1 case. The tumor sizes ranged from 4.2 to 4.9 cm (median 4.5 cm) on MR axial imaging. On T 1WI, 5 cases showed isointense compared with adjacent temporal muscles, with focal hypointense in 2 cases. On T 2WI, the tumor was graded as isointense in 3 cases, and hyperintense in 2 cases. Heterogeneous enhancement in all cases with mild in 3 cases, and moderate in 2 cases on postcontrast MR imaging. On DCE-MRI of 5 cases, there were 3 cases of type Ⅲ (washout-shaped curves), and 2 cases of type Ⅱ of the TIC (plateau-shaped curves). The range of ADC values was from 0.63×10 -3 to 1.17×10 -3 mm 2/s, and median ADC value was 0.84×10 -3 mm 2/s, of 5 cases with varying degrees of high signal on DWI. The Ki-67 index ranged from 30% to 80% of the tumor. An immunohistochemical study showed that the tumor cells of 5 cases were all positive for both NUT and INI-1 genes. One case was performed with biopsy and followed by chemotherapy, four cases were performed with surgery, combined with the following chemotherapy, and one also was implemented with radiation therapy. The follow-up time was 7-16 months. Five cases were all alive during the follow-up. Conclusions:The NUT midline sinonasal tract carcinoma is a rare, gene-related solid malignant tumor. The tumor is more commonly seen in young patients, mostly centered in the nasal and ethmoid region with invasive growth, more calcification on CT, and heterogeneous enhancement on MRI. These findings are some characteristics of the tumor.

ABSTRACT

Objective:To explore the application value of apparent diffusion coefficient (ADC) of magnetic resonance diffusion weighted imaging (DWI) parameters in glioma classification and glioma microstructure evaluation.Methods:From June 2017 to November 2019, 38 patients with glioma confirmed by surgery and pathology in Haikou Hospital Affiliated to Xiangya Medical College of Central South University were retrospectively analyzed. According to the pathological results, they were divided into low-grade (WHO Ⅰ-Ⅱ, 15 cases) glioma group and high-grade (WHO Ⅲ-Ⅳ, 23 cases) glioma group. They received magnetic resonance imaging (MRI) plain scan and DWI scan respectively, and the ADC value and microstructure of different grades of glioma were compared. The correlation between ADC value of glioma and the percentage of vascular endothelial growth factor (VEGF)-positive cells, cell density and integrated optical density (IOD) value of aquaporin 1 (AQP1) expression was analyzed.Results:(1) MRI examination showed that the signals of low-grade glioma were more uniform, with no or slight peritumoral edema and space occupying effect, and the enhancement was more non enhanced or slightly enhanced. The signals of high-grade glioma were more heterogeneous due to necrosis and bleeding, and the peritumoral edema and space occupying effect were more obvious, showing uneven obvious enhancement or irregular ring enhancement; (2) The percentage of VEGF positive cells, cell density and the IOD value of AQP1 expression in high-grade glioma were significantly higher than that in low-grade glioma, and the ADC value was lower than that in low-grade glioma (all P<0.05); (3) The ADC value of glioma patients was negatively correlated with the percentage of VEGF-positive cells, cell density, and the IOD value of AQP1 expression ( r=-0.55, -0.65, -0.63, all P<0.05). Conclusions:The ADC value of glioma can indirectly reflect the expression of VEGF, cell density and AQP1 positive expression level, which is helpful for preoperative glioma classification and evaluation of glioma microstructure and biological characteristics.

ABSTRACT

A blood-brain barrier microfluidic chip platform for studying the permeability of active components in traditional Chinese medicine was developed. This model used primary human brain microvascular endothelial cells on a microfluidic chip consisting of two perpendicularly-crossing channels and a single layer porous polycarbonate membrane. The physiological shear stress in the human vasculature was also modeled in this device. Cell viability on the chip was monitored by cell staining and immunofluorescence staining. The cells spread well and the structure of an intercellular adhesion protein was satisfactory. The permeability of fluorescent tracers and three model drugs and the functional expression of P-glycoprotein (P-gp)on the blood-brain barrier were investigated. The results show that the apparent permeability coefficients (Papp) of the fluorescent tracers and three model drugs were consistent with those reported in the literature, and P-gp on the chip showed normal function, indicating that there was a complete structure and a functional BBB. The permeability of six active components of traditional Chinese medicine was investigated through this microfluidic chip and the drug concentration was determined by HPLC-MS/MS to obtain the Papp of each component. The Papp of corydaline was (4.51 ± 1.90)×10-7 cm·s-1, the Papp of tetrahydropalmatine was (9.10 ± 6.59)×10-7 cm·s-1, and the Papp of imperatorin was (9.38 ± 2.53)×10-7 cm·s-1; the concentration of isoimperatorin, baicalin and chlorogenic acid was below the limit of quantification, which suggested that isoimperatorin, baicalin and chlorogenic acid have poor permeability in this BBB chip. This blood-brain barrier microfluidic platform possesses a complete barrier function and near-physiological conditions and could be a valuable in vitro tool for drug permeability evaluation.

ABSTRACT: We search for positive utility impact on the invasive species Pterygoplichthys spp. The optimal level of replacement of sardine meal (SM) by Pterygoplichthys spp. meal (PLM) in practical feeds for fry of Nile tilapia (Oreochromis niloticus) was evaluated. We evaluated six experimental diets: 50% PLM-50% SM, 60% PLM-40% SM, 70% PLM-30% SM, 80% PLM-20% SM, 90% PLM-10% SM and 100% PLM-0% SM. In a recirculation system, 270 sex-reversed tilapia fry were used (0.7 ± 0.1 g). Diets were administered in triplicate and the experiment lasted 56 days with sampling every 14 days. There were no statistical differences in growth. At the end of the experiment, the SGR, ADG, CF, WG, DFI, FCR and PER were determined without significant differences (P > 0.05) between treatments, but there was a tendency that could indicated higher WG and PER in the 90% PLM-10% SM treatment (93.56 ± 0.43% and 8.44 ± 1.18 g, respectively). All survival rates were higher than 90% with no significant differences. Additionally, there were no statistical differences in the chemical composition of the whole fish, while apparent digestibility coefficients of protein (81-90%) and lipids (87-93%) also showed no significant differences. We concluded that 100% PLM can replace 100% SM in feeding fry of O. niloticus without affecting their survival, growth or chemical composition or the apparent digestibility of nutrients.


RESUMO: Buscamos por um impacto positivo de utilidade nas espécies invasoras como Pterygoplichthys spp. Foi avaliado o nível ótimo de substituição da farinha de sardinha (SM) pela farinha de Pterygoplichthys spp. (PML) em dietas práticas para tilápia do Nilo (Oreochromis niloticus). Foram utilizadas seis dietas experimentais; 50% PLM-50% SM, 60% PLM-40% SM, 70% PLM-30% SM, 80% PLM-20% SM, 90% PLM-10% SM e 100% PLM-0% SM. Em sistema de recirculação foram colocados 270 filhotes de tilápia machos (0.7 ± 0.1 g). As dietas foram triplicadas, o experimento durou 56 dias com amostragem a cada 14 dias, não houve diferença estatística para o crescimento. No final do experimento, foram determinados SGR, ADG, CF, WG, DFI, FCR e PER, não encontrando diferenças significativas (P> 0,05), embora tenha havido uma tendência indicando maior WG e PER na dieta 90% PLM-10% SM (93,56 ± 0,43% e 8,44 ± 1,18 g). A sobrevida foi superior a 90%, sem diferenças significativas. Além disso, não houve diferenças significativas na composição proximal de todo o peixe, enquanto o aparente coeficiente de digestibilidade de proteínas (81-90%) e lipídios (87-93%) também não mostrou diferenças significativas. Podemos concluir que o uso de 100% de farinha de Pterygoplichthys spp. pode substituir 100% de farinha de sardinha na alimentação de jovens O. niloticus sem afetar a sobrevivência, o crescimento, a composição química e a digestibilidade aparente dos nutrientes.
